  • 7 am, every other weekend
Shift Differential:
$3.00 extra for overnights! Northgate Health Care Facility is looking to add an RN Unit Coordinator to its nursing leadership team. In this role, you'll oversee the clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, staff supervision, and interdisciplinary coordination. RESPONSIBILITIES
Oversee the clinical care and daily operations of an assigned nursing unit Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff Conduct resident assessments and ensure care plans are implemented, updated, and followed appropriately Monitor resident conditions and communicate changes with providers, families, and interdisciplinary team members Review provider orders, laboratory results, incident reports, and quality indicators Participate in care plan meetings, family meetings, QAPI activities, and interdisciplinary rounds Coordinate admissions, discharges, transfers, outside appointments, and resident services Ensure accurate documentation, medication management, and regulatory compliance Support staff education, orientation, coaching, and performance management activities Monitor infection control, immunization tracking, resident safety, and quality improvement initiatives Address resident, family, and staff concerns while promoting a positive care environment Perform additional duties as assigned
REQUIREMENTS
Associate Degree in Nursing required; Bachelor's Degree preferred Valid NYS Registered Nurse (RN) License required Minimum 1 year of RN experience in a long-term care setting required 3-5 years of long-term care experience with supervisory experience preferred Strong knowledge of resident assessments, care planning, and long-term care regulations Leadership experience with the ability to supervise, coach, and mentor staff Excellent communication, organizational, and problem-solving skills Ability to coordinate care across multiple disciplines and manage competing priorities Experience with quality improvement, infection prevention, and regulatory compliance preferred Commitment to providing compassionate, resident-centered care
