Responsibilities:
Perform comprehensive home health risk assessments tailored to Medicare Advantage members Review and evaluate prior/current medical history, ongoing conditions, and medication usage Offer patient-focused education related to conditions and prescribed medications Detect gaps in care and recommend actionable, relevant next steps This role does not require creating treatment plans or prescribing medications
Qualifications:
