Schedule:

Flexible shifts available seven days a week You choose when and how often you work Minimum hourly commitment required each week or every other week Responsibilities include: Conduct comprehensive home-based risk assessments, typically lasting 45 minutes per visit Review prior and current medical history, health conditions, and medication regimens Educate patients and their families on health conditions, preventive measures, and medication management Identify care gaps and recommend actionable next steps for ongoing health Focus solely on assessmentno treatment planning or prescribing duties required

Qualifications:

Active Nurse Practitioner license in Ohio Board certified by AANP or ANCC Current Basic Life Support (BLS/CPR) certification At least 6 months of recent NP experience is preferred Master of Science in Nursing (MSN) from an accredited nursing program This position offers a competitive rate per completed evaluation, along with mileage reimbursement and compensation for administrative work.
