MINIMUM QUALIFICATION

Education, Knowledge, Skills and Abilities Degree in nursing required, BSN preferred. Strong clinical and assessment skills Ability to work independently Strong interpersonal communication skills Ability to document in an EMR Required Length and Type of Experience One year nursing experience preferred New graduates welcome -Individualized Orientation Program with Mentor program for new graduates Required Licensure or Certification Current State of Ohio Registered Nurse license Current Valid State Driver's License (VDL) Minimum of two years of driving experience; minimum of five years of driving experience if transporting patients and/or visitors. Zero points preferred on driver's license and cannot exceed six points total allowable. Maintain evidence of personal auto insurance. Assume responsibility to pay fees associated with traffic violations. Must maintain eligibility of insurability as determined by ARMC's insurance carrier's requirement to operate organization's vehicles. Required Physical and Environmental Demands Requires eye/hand coordination and manual dexterity sufficient to operate a telephone, analyze and prepare written records, and provide hands-on care to clients. Requires normal range of hearing to communicate with staff, clients, physicians, and other health care providers. Must be able to lift and carry 25 lbs.

GENERAL SUMMARY

The Home Health RN acts as a primary care manager for a caseload of patients. The RN's role is to assess, plan, implement, and evaluate the plan of care coordinating with the patient's physician. The case manager functions in an assigned territory within Ashtabula County.
