The Nurse Case Manager is responsible for the daily coordination and management of clients' medical, dental, and behavioral health needs at Safely Home, Inc. This position serves as the primary point of coordination for routine client healthcare needs, including scheduling and attending appointments, communicating with healthcare providers, maintaining medical documentation, coordinating medications and prescriptions, and ensuring appropriate follow-up care is completed. The Nurse Case Manager works closely with the On-Call Nurse, residential and clinical staff, healthcare providers, pharmacies, custodians, and other members of the interdisciplinary team to ensure continuity of care and timely response to client medical needs. The Nurse Case Manager also assists with the medical component of client admissions, intakes, transitions, and discharges, including gathering and reviewing medical information, identifying immediate healthcare needs, coordinating medications and prescriptions, and helping ensure necessary medical documentation is obtained and maintained.ESSENTIAL DUTIES
Medical Care Management and Coordination- Coordinate the day-to-day medical, dental, and behavioral health needs of clients.
- Schedule routine, follow-up, specialty, dental, vision, behavioral health, and other healthcare appointments as needed.
- Transport and accompany clients to medical and healthcare appointments as assigned.
- Maintain an organized system for tracking upcoming appointments, follow-up appointments, referrals, laboratory work, testing, and other healthcare recommendations.
- Ensure recommended follow-up care is scheduled and completed within appropriate timeframes.
- Communicate relevant appointment information, provider recommendations, medication changes, restrictions, and follow-up instructions to the appropriate agency staff.
- Maintain ongoing communication with healthcare providers, pharmacies, the On-Call Nurse, and other members of the interdisciplinary team regarding client healthcare needs.
- Assist with obtaining prescriptions, refills, medical supplies, and other healthcare-related items.
- Coordinate prescription pickup and pharmacy communication as necessary.
- Monitor client medical needs and ensure emerging concerns are communicated promptly to the On-Call Nurse or appropriate healthcare provider.
- Ensure client medical care remains current, including routine physicals, dental care, vision care, immunizations, and other required or recommended healthcare services.
- Maintain awareness of each client's ongoing medical needs, diagnoses, allergies, medications, appointments, restrictions, and recommended follow-up care within the scope of the position. Medication Management
- Administer medications in accordance with agency policy, physician orders, and applicable state requirements.
- Maintain accurate and timely medication administration documentation.
- Assist with coordinating medication orders, refills, pharmacy needs, and medication changes.
- Ensure medication-related documentation and physician orders are obtained and maintained as required.
- Communicate medication concerns, missed doses, medication changes, side effects, or other medication-related issues to the On-Call Nurse and appropriate staff.
- Work closely with the On-Call Nurse to promote safe and consistent medication management practices. Medical Intake, Admission, Transition, and Discharge Responsibilities
- Assist with the medical portion of client intake and admission procedures.
- Review available medical histories, current diagnoses, allergies, medications, physician orders, healthcare providers, and other relevant healthcare information upon admission.
- Identify missing medical information or documentation and assist with obtaining needed records from custodians, healthcare providers, pharmacies, or other appropriate sources.
- Assist with ensuring medications and medication orders are available and appropriately coordinated at the time of admission.
- Identify immediate medical, dental, behavioral health, or medication needs and communicate them to the On-Call Nurse and appropriate members of the treatment team.
- Assist with coordinating initial healthcare appointments when required following admission.
- Participate in medical care transitions when clients move between placements or levels of care.
- Assist with the medical component of discharge planning, including medication coordination, upcoming appointment information, healthcare records, and continuity-of-care needs. Medical Records and Documentation
- Maintain accurate, complete, and timely medical documentation in accordance with agency policy and applicable requirements.
- Document healthcare appointments, provider recommendations, follow-up instructions, medication-related information, and other relevant medical information.
- Maintain appointment records and supporting healthcare documentation within the appropriate client record.
- Ensure updated medical information is communicated and available to appropriate staff.
- Assist with obtaining and maintaining medical records, physician orders, discharge summaries, laboratory results, immunization records, dental records, and other healthcare documentation as applicable.
- Complete required incident reports, medical documentation, appointment documentation, and related compliance forms in a timely manner.
- Assist with monitoring medical records for missing, expired, or outdated healthcare documentation and follow up as necessary. Collaboration and Communication
- Work collaboratively with the On-Call Nurse regarding client healthcare needs and changes in medical status.
- Communicate professionally with physicians, dentists, behavioral health providers, pharmacies, hospitals, urgent care facilities, custodians, parents or guardians when applicable, and other community providers.
- Coordinate with residential, clinical, educational, and administrative staff to support continuity of care.
- Communicate significant changes in a client's health, medication, treatment recommendations, or medical restrictions to appropriate staff.
- Participate in interdisciplinary meetings when medical information or coordination is needed.
- Support positive and professional relationships with community healthcare providers and internal program staff. Confidentiality and Ethics
- Maintain confidentiality of client medical and personal information in accordance with HIPAA, applicable state and federal requirements, and agency policies.
- Protect medical records and healthcare information from unauthorized access or disclosure.
- Maintain professional and ethical conduct in all interactions with clients, staff, healthcare professionals, custodians, and community partners. Other Duties
- Perform additional medical coordination or program-related duties as assigned by the On-Call Nurse, Executive Director, or designee.
- Assist with other responsibilities necessary to support client health, safety, and continuity of care.
QUALIFICATIONS
- Current, active Licensed Practical Nurse (LPN) license in the State of Ohio preferred.
- Experience in behavioral health, residential treatment, pediatric/adolescent care, or a related healthcare setting preferred.
- Knowledge of medication administration, medication documentation, and general nursing practices.
- Ability to coordinate medical, dental, behavioral health, and other healthcare services for multiple clients.
- Ability to communicate effectively with physicians, pharmacies, healthcare providers, guardians/custodians, agency nursing staff, and other members of the interdisciplinary team.
- Strong organizational and time-management skills, including the ability to schedule and track appointments, follow-up care, medication needs, medical orders, and required documentation.
- Ability to work independently while collaborating effectively with agency leadership, residential staff, clinical staff, and outside healthcare providers.
- Strong written and verbal communication skills with attention to accuracy and detail.
- Familiarity with regulatory and documentation requirements applicable to residential treatment and healthcare environments.
- Experience working with children.
ADDITIONAL REQUIREMENTS
- Must be at least 21 years of age.
- Must pass required BCI/FBI background checks and physical examination.
- Must possess a valid driver's license with an acceptable driving record.
- Must be able and willing to transport clients to healthcare appointments as required.
- Must maintain required Crisis Intervention Certification (e.g., CPI).
- Must maintain required medication administration and other position-specific certifications or training.
- This is a 12-month, full-time, on-site position with extended hours and local travel as needed to meet client healthcare needs.
PHYSICAL AND WORK ENVIRONMENT REQUIREMENTS
- Must be able to walk, stand, bend, and kneel throughout the workday.
- Ability to lift up to 25 pounds and perform CPI physical interventions as required and trained.
- Regular local travel is required for client medical, dental, behavioral health, and other healthcare appointments.
- Occasional lifting, carrying, pushing, and pulling of equipment or supplies up to 25 pounds.
- Potential exposure to blood and bodily fluids.
EXPECTED HOURS
- Monday through Friday, 8:00 a.m. - 4:00 p.m.
- Flexibility permitted with supervisory approval.
- Evening or adjusted hours may occasionally be required based on client appointments or healthcare needs.
