Key Responsibilities:

Conduct comprehensive annual health risk assessments delivered in members homes Evaluate patients medical history, ongoing conditions, and medication usage Educate individuals on their diagnoses and medications to encourage better health outcomes Identify care gaps, recommend next steps, and help patients navigate their health journey Documentation and assessment onlyno prescribing or development of treatment plans required

Qualifications:

Active Nurse Practitioner license in Ohio Current Board Certification (AANP or ANCC) Valid CPR certification (BLS) Minimum of 6 months NP experience preferred MSN degree from an accredited nursing program Compensation for each evaluation ranges from $80-$90, and this role includes a mileage stipend and payment for administrative time.
