PM Key Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans for each patient. Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and management of chronic diseases. Monitor patient conditions, identify changes, and communicate promptly with the care team and physicians. Provide patient and family education on treatment plans, medications, and preventive care. Ensure accurate and timely documentation using Home Care Home Base (HCHB). Collaborate with physical therapists, occupational therapists, social workers, and aides for coordinated patient care. Take part in case conferences, staff meetings, and ongoing education sessions to support continuous improvement. Adhere to all agency and regulatory standards related to documentation and patient care.

Qualifications:

Active and unencumbered RN License in the state of practice. Minimum 1 year of recent home health experience; field-based experience is strongly preferred. Proficiency in Home Care Home Base (HCHB) is required. Demonstrated experience with OASIS. Reliable transportation, a valid drivers license, and current auto insurance are necessary.
