Full-time | Fri 4 PM-8 AM/ Sat 8 PM-8 AM/ Sun 8 PM-8

AM Location:

Hamilton County, Ohio, and surrounding areas Requirements of the After Hours RN role: Licensed RN in the state of practice BLS Certified A minimum of one year of skilled nursing care experience providing comprehensive patient care in a post-acute setting. Valid driver's License and Auto insurance Coverage Reliable transportation: Willing to travel On-call/ triage experience preferred About the After Hours RN ro le: Provide on-call services after hours and complete skilled emergency and scheduled visits. Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. Triage calls, coordinates services, and provides frequent updates on plan of care and any changes in condition. Assist reinforcement of patient and family education regarding end-of-life care. Document patient care and clinical information accurately using an electronic medical record (EMR) system on a mobile device Participate in IDG and case conferences when needed Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. Keep the RNCM informed of changes noted during the process. Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
