We're looking for an experienced RN Case Manager to join our Home Health team on the West Side of Cleveland , offering top-quartile pay, a $5,000 sign-on bonus, and the tools and support to do your best work every day. Compensation $39.00

Requirements Must-Have:
Active RN license (Ohio) + current BLS certification Associate or Bachelor's degree in Nursing (BSN preferred) Minimum 2 years of clinical nursing experience, including at least 1 year in a home health setting Reliable vehicle and valid driver's license with ability to travel throughout the West Side Cleveland service area Familiarity with Medicare and Medicaid regulations Proficiency in electronic medical record (EMR) systems and home health-specific software Preferred /
Nice-to-Have:
OASIS certification or demonstrated OASIS experience Familiarity with HCHB or similar home health EMR Strong caseload management and time management skills Experience with care coordination across interdisciplinary teams
