Brooklyn, OH Job Details 1 day ago Qualifications CPR Certification RN License Full Job Description McGregor PACE (Program of All-Inclusive Care for the Elderly), is a community healthcare service program that provides services to the elderly as an alternative to nursing home placement, allowing seniors to remain at home, with support of our Adult Day Health Center We are expanding to Summit County and looking for an experienced Homecare Nurse who will deliver comprehensive health services for program participants. This candidate will work closely with our primary care physician and PACE interdisciplinary team.
Work Schedule:
Monday-Friday, no weekends! Major holidays off
Responsibilities:
Works collaboratively with the Interdisciplinary Team, Clinic Manager and Site Director to deliver efficient quality care. Assists in coordination of services with other McGregor PACE departments. Works collaboratively with the other departments to identify the learning needs of departmental personnel and makes recommendations for/participates. Provides nursing care, including assessing and monitoring health status, functional status response to treatment, medication administration. Identifies participant and family education needs and provides appropriate teaching/information. Assists in departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant.
Minimum Qualifications:
Registered nurse; licensed to practice in the state of Ohio Reliable transportation CPR certification required
Preferred Qualifications:
Bachelor's degree in nursing preferred Minimum one of clinical experience working with the elderly preferred Previous experience in an interdisciplinary care setting, desirable
