Each assessment typically takes about 45 minutes and may involve an annual health evaluation or a post-discharge visit, allowing for meaningful one-on-one patient interactions.

Key Responsibilities:

Complete comprehensive annual health risk assessments in patients homes Review and assess prior and current medical history, conditions, and medications Educate patients on their health conditions and prescribed medications Identify care gaps and make actionable recommendations

Required Qualifications:

Active Nurse Practitioner License in Ohio Current BLS/CPR certification Board Certification through AANP or

ANCC MSN
