Schedule:

Full-time, Monday through Friday On-call responsibilities are probable Daily visit expectations are clearly structured

Key Responsibilities:

Deliver in-home nursing care and skilled clinical interventions Perform thorough patient assessments and timely, accurate OASIS documentation Collaborate with physicians and multidisciplinary care teams to develop and implement tailored care plans Execute wound care, manage medications, provide patient and family education, and monitor disease progression Maintain consistent communication regarding patient status changes to ensure optimal outcomes

Qualifications:

Active, unrestricted RN license At least 1 year of recent home health RN experience Demonstrated proficiency with OASIS assessments (required) Experience with HomeCare HomeBase (strongly preferred) Ability to manage a full-time, structured schedule with daily patient visits Reliable transportation and a valid drivers license
