Role:
As an MDS Nurse at Aspen Glen Nursing and Rehabilitation Center, you will play a vital role in our interdisciplinary team, responsible for conducting comprehensive assessments, developing individualized care plans, and ensuring the delivery of high-quality care to our residents. With a focus on patient-centered care, you will work closely with our experienced healthcare professionals to provide personalized attention, support, and education to our residents and their families.
Responsibilities:
Conduct initial and ongoing assessments of residents using the Minimum Data Set (MDS) assessment tool Develop and implement individualized care plans, in collaboration with the interdisciplinary team Provide direct patient care, including wound care, medication administration, and vital sign monitoring Assess resident needs and resources, and make recommendations for optimal care delivery Collaborate with healthcare providers, therapists, and other team members to ensure comprehensive care Maintain accurate and detailed records of assessments, care plans, and resident progress Participate in quality improvement initiatives and maintain compliance with regulatory requirements
Requirements:
Current licensure as a Registered Nurse (RN) in the state of Ohio 1+ year experience in an MDS role 0 years of experience in gerontological nursing or a related field Strong clinical assessment and documentation skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment and prioritize tasks efficiently
