Full-time, Monday through Friday On-call responsibilities likely

Key Responsibilities:

Deliver in-home skilled nursing care to patients with varying needs Complete thorough and accurate patient assessments, including timely OASIS documentation Collaborate with physicians and care coordinators to ensure personalized care plans Administer wound care, medication management, patient and family education, and ongoing disease monitoring Communicate patient status updates and changes in condition promptly to the care team

Desired Qualifications:

Current, active RN license Minimum of one year of recent home health experience as an RN Proven proficiency with OASIS documentation (required) Experience with HomeCare HomeBase (highly preferred) Strong organizational and time management skills to handle structured daily visits Reliable transportation and valid drivers license
