Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members in the community Evaluate a patients prior and current medical history, ongoing conditions, and medication regimens Deliver clear education on health conditions and prescribed medications Identify gaps in care and suggest actionable recommendations to enhance patient well-being

Qualifications:

Active Nurse Practitioner license in Ohio Current CPR certification (BLS) Board certification via AANP or ANCC Preferred minimum of six months of current NP experience MSN degree from an accredited nursing program This position offers compensation at $80$90 per completed evaluation, in addition to a mileage stipend and paid administrative time, making your expertise and dedication truly worthwhile.
