Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

This role is ideal for an experienced RN who is comfortable working independently, managing patient needs in the home, and supporting quality care with professionalism, urgency, and empathy. $100.00 per day What We're Looking For We are seeking a dependable Registered Nurse who is confident in home health and hospice care, comfortable working autonomously, and committed to delivering patient-centered care in a home-based setting. The ideal candidate is clinically strong, organized, calm under pressure, and able to communicate effectively with patients, families, physicians, and the interdisciplinary team. Candidates should be comfortable assessing patient needs, coordinating care, supporting comfort and safety across home health and hospice services, and maintaining accurate, timely documentation.
Requirements:
Current Registered Nurse (RN) license in the state of practice. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid driver's license and reliable transportation for home visits. Current CPR certification. Experience with electronic medical record systems and documentation preferred. Physical Requirements Lift and transfer patients and carry equipment or supplies up to 50 pounds Stand, bend, squat, and walk for extended periods during home visits Use fine motor skills for documentation and patient care tasks Respond swiftly and efficiently to urgent or emergency situations Maintain regular attendance and timeliness Work calmly and effectively in high-stress situations
