The ideal candidate will possess a comprehensive understanding of patient care, case management, and clinical workflows within diverse healthcare settings. As a Registered Nurse Case Manager, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure optimal health outcomes through effective utilization management and discharge planning. Your expertise will contribute to delivering high-quality, patient-centered care across inpatient, outpatient, and post-acute environments. Responsibilities Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine appropriate care pathways. Develop individualized care plans that align with clinical guidelines and patient needs, incorporating discharge planning and post-acute care strategies. Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate treatment plans and optimize patient outcomes. Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment progress to ensure appropriate resource utilization. Facilitate intake procedures for new patients while maintaining compliance with HIPAA regulations to protect patient confidentiality. Monitor patient progress during hospitalization or outpatient visits, adjusting care plans as necessary based on clinical findings and patient response. Assist in managing complex cases involving infectious disease care, behavioral health issues, geriatrics, or trauma cases such as Level I or Level II trauma centers. Support discharge planning by coordinating services in post-acute care settings or outpatient clinics to ensure seamless transitions of care. Skills Extensive critical care experience with a strong background in ICU, emergency medicine, or trauma center environments. Proven expertise in hospital medicine, medical-surgical units, or specialized areas such as PICU or OR settings. Knowledge of physiology principles and diagnostic evaluation techniques essential for comprehensive patient assessment. Familiarity with managed care principles, utilization management protocols, and medical records documentation standards. Strong understanding of HIPAA regulations along with medical terminology relevant to acute care and outpatient services. Excellent communication skills for effective collaboration with multidisciplinary teams and clear patient education. Ability to handle complex cases involving geriatrics, behavioral health issues, or infectious diseases while maintaining high standards of patient service and safety.
