Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. This role works as part of the clinical provider team, delivering a wide range of direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans. The ideal candidate will collaborate with physicians and healthcare staff to provide high-quality, patient-centered care and drive positive health outcomes.
Key Responsibilities:
Provide high-quality patient care through the diagnosis and treatment of acute and chronic conditions Obtain and review patient histories (medical, developmental, social, and family) and perform comprehensive physical exams Order, perform, and interpret diagnostic tests and preventive screenings Develop and manage treatment plans, including prescribing medications and educating patients on conditions and care Promote preventive care through immunizations, screenings, and patient education on health, nutrition, safety, and disease prevention Document all patient encounters accurately, including history, exam findings, diagnoses, and care plans Review and maintain patient records, including labs, imaging, medications, and immunization status Coordinate care by facilitating referrals and collaborating with healthcare team members Monitor patient progress, follow up on test results, and manage high-risk patient population.
Qualifications:
Education Master's or Doctoral degree in Nursing Certified Nurse Practitioner licensed in the State of Ohio Work Experience Minimum of 5 years of experience in family medical practice FNP preferred, with a strong desire to work with children. Experience in a physician office, ambulatory care, or clinical health setting preferred Qualifications Board certification as a Nurse Practitioner Active DEA license (or ability to obtain) Strong clinical decision-making and communication skills Commitment to patient-centered care
Preferred Qualifications:
Experience in primary care, family medicine, or community health Bilingual skills (if applicable) Familiarity with electronic health record (EHR) systems
