Job Summary:

On Call

• one day per week and every 4th weekend Delivers direct/indirect patient care to adults and geriatric population Assesses/reassesses patient's condition based upon observation of patient and interview of patient and/or family, physician's diagnosis and data, diagnostic results, and relevant data from other healthcare providers Evaluates/re-evaluates patient's response to rehab nursing interventions and patient's progress to expected outcome(s) and community discharges.

Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care. Provides patients and family members with relevant health care education and instructions regarding treatments and procedures Requirements Include Graduate of an accredited program for nursing education, BSN preferred. Current licensure to practice professional nursing in the State of Ohio. Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required. ACLS required RN
