Take the lead in delivering high-quality patient care as an experienced Registered Nurse specializing in home health. This full-time, 13-week contract offers the chance to work directly with patients in their homes, providing essential nursing services and ongoing support throughout the Niles, OH community.

Schedule:

Monday Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (typically 35 patient visits per day) Field-based position requiring daily travel within the assigned territory

Qualifications:

Current, active RN license in good standing for the state of practice At least 1 year of recent field-based Home Health experience Proficient with Home Care Home Base (HCHB) documentation system OASIS assessment expertise Reliable transportation, valid driver's license, and active auto insurance

Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, recognize changes, and escalate care concerns as needed Provide thorough education for patients and families on diagnoses, treatments, medications, and preventive health strategies Document care accurately in HCHB, ensuring timely and compliant recordkeeping Collaborate closely with interdisciplinary team membersincluding therapy staff and social workersfor coordinated care delivery Attend case conferences, staff meetings, and participate in ongoing education Meet all regulatory, compliance, and quality standards in home health practice
