Ensure accurate and timely submission of MDS documentation. Collaborate with interdisciplinary teams to develop and update resident care plans. Serve as a resource for nursing staff regarding MDS and care planning processes. What We're Looking For Current RN or LPN license in good standing. Previous MDS experience in a skilled nursing or long-term care setting preferred. Strong knowledge of Medicare, Medicaid, and PPS processes. Excellent organizational and communication skills.
