Job Overview The Registered Nurse (RN) provides clinical oversight, care coordination, and support for home health and waiver services delivered by the agency. This role ensures high‑quality patient care, regulatory compliance, accurate documentation, and effective communication between caregivers, physicians, case managers, and payers. The RN works in the office but may complete occasional field visits when needed. Key Responsibilities Clinical Oversight & Care Coordination Review, approve, and update patient care plans, service notes, and clinical documentation. Monitor patient conditions, changes in status, and ensure timely communication with physicians and case managers. Provide clinical guidance to aides, LPNs, and caregivers regarding patient needs and care expectations. Conduct supervisory visits as required by Ohio Medicaid, MyCare Ohio, and Medicare guidelines. Ensure all services delivered meet medical necessity and regulatory standards. Documentation & Compliance Complete and review nursing assessments, supervisory notes, incident reports, and care plan updates. Ensure documentation meets payer requirements (Medicaid, MyCare, Medicare Advantage). Maintain compliance with agency policies, ODH regulations, and accreditation standards. Assist with chart audits, QA reviews, and corrective action plans. Communication & Coordination Serve as the primary clinical contact for case managers, physicians, and family members. Communicate changes in care plans, authorizations, and patient needs to office staff and caregivers. Collaborate with scheduling staff to ensure appropriate staffing and continuity of care. Training & Support Provide training and competency checks for aides and LPNs. Educate staff on clinical procedures, documentation standards, infection control, and safety protocols. Support onboarding of new caregivers and office staff. Office-Based Responsibilities Review EVV records for accuracy and compliance. Assist with authorization management, including submission of clinical documentation. Participate in interdisciplinary team meetings and care conferences. Support billing and administrative staff by clarifying clinical details when needed. Qualifications Current Registered Nurse (RN) license in the State of Ohio. Minimum 1 year of nursing experience (home health or long‑term care preferred). Strong understanding of Ohio Medicaid, MyCare Ohio, and Medicare home health requirements. Excellent communication, documentation, and organizational skills. Ability to work independently and manage multiple priorities in a fast‑paced office environment. Proficiency with EMR systems, EVV platforms (Sandata), and basic computer applications. Schedule & Work Environment Full‑time, office‑based position with occasional field visits. Monday-Friday daytime hours. Collaborative, supportive team environment.
Pay:
$28.00 - $32.00 per hour Expected hours: 37.5 per week
