Job Overview We are seeking a highly motivated and detail-oriented MDS (Minimum Data Set) Coordinator to join our dynamic healthcare team. Duties Coordinate and facilitate the completion of the MDS assessments in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines, ensuring timely submission. Review medical records, clinical documentation, and interdisciplinary notes to ensure accuracy, completeness, and compliance with clinical documentation standards. Collaborate with nursing staff, physicians, case managers, and other healthcare professionals to gather pertinent information for accurate MDS coding. Ensure proper utilization of EMR (Electronic Medical Records) systems such as Cerner, Epic, or Meditech to document resident data efficiently. Perform thorough documentation reviews to identify gaps or inconsistencies that could impact reimbursement or quality metrics like HEDIS or NCQA standards. Maintain up-to-date knowledge of long-term care regulations, Medicare policies, ICD-10/ICD-9 coding standards, CPT coding, DRG (Diagnosis-Related Group) classifications, and HIPAA requirements. Support discharge planning, hospice care coordination, and post-acute care transitions by providing accurate clinical data. Conduct ongoing education for staff on clinical documentation improvement initiatives and regulatory updates related to CMS guidelines. Experience Proven experience in long-term care facilities such as nursing homes or skilled nursing facilities with familiarity in managing MDS processes. Strong background in medical office procedures with proficiency in EMR/EHR systems like Cerner, Athenahealth, or eClinicalWorks. In-depth knowledge of Medicare regulations, HEDIS standards, NCQA accreditation requirements, and state healthcare policies. Critical care experience in ICU or emergency medicine settings is highly desirable for understanding complex patient needs. Demonstrated expertise in medical coding including

ICD-10/ICD-9

coding systems, CPT coding, DRG classification, and utilization management practices. Familiarity with hospital inpatient experience or Level I/II trauma center procedures enhances understanding of acute care documentation standards. Excellent communication skills combined with a thorough understanding of anatomy, physiology, nursing practices, and medical terminology to accurately interpret clinical data.
