Work Monday through Friday, 8:00 AM to 5:00 PM, providing essential skilled nursing services in patient homes. Expect a caseload of approximately 30 points per weekgenerally 35 patient visits dailysupported by a robust electronic documentation system (Home Care Home Base) and the OASIS assessment tool. Daily travel is required within your assigned territory, with mileage reimbursement included.

Key Responsibilities:

Complete thorough OASIS assessments and develop personalized care plans for each patient. Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and ongoing management of chronic conditions. Monitor patient status, recognize significant changes, and promptly communicate with the interdisciplinary care team and physicians. Educate patients and their families on important health topics, medications, disease processes, and preventive strategies. Accurately document all care and communications in Home Care Home Base (HCHB) in a timely manner. Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure comprehensive, coordinated care. Engage in case conferences, staff meetings, and participate in ongoing professional education. Uphold all regulatory and organizational standards for clinical documentation and patient care.

Qualifications:

Current, active RN license (state of practice), in good standing. At least one year of recent home health nursing experience; field experience highly desirable. Strong proficiency with Home Care Home Base (HCHB) and OASIS required. Reliable vehicle, valid drivers license, and current auto insurance.
