Full-Time, in person Make a Difference in Long-Term Care Valor Clinical Partners is looking for an RN or LPN Case Manager to join our clinical team. In this role, you will work in our long-term care facilities to help residents get the care they need. You will work closely with facility nurses, NPs, physicians, residents, and families. Our Case Managers help find health concerns early, coordinate care, and prevent avoidable hospital stays. What You'll Do Visit residents in assigned long-term care facilities. Follow residents with complex or changing health needs. Watch for changes in condition and help address concerns early. Work with facility nurses, NPs, physicians, and other care team members. Help coordinate appointments, testing, treatments, and follow-up care. Follow up after ER visits and hospital stays. Help residents return safely to the facility after a hospital stay. Help identify ways to prevent avoidable hospital transfers. Support residents and families with care questions. Keep accurate and timely documentation. Build strong working relationships with facility teams. Take part in quality and care improvement efforts. What We're Looking For Active Ohio RN or LPN license in good standing. Valid driver's license and reliable transportation. Ability to travel to assigned facilities. Experience in long-term care, skilled nursing, geriatrics, or a similar setting preferred. Strong communication and organization skills. Able to work independently and manage your daily schedule. Comfortable working with older adults and complex health needs. $62,000.00 - $72,000.00 per year
