Job Title:
RN Case Manager Assignment Length:
13 weeks with possible extension
Location:
Chillicothe, OH Shift:
Days, M-F, 730a-4p
Pay Rate:
$65/hour
Job Summary:
Assessment of discharge needs, Social determinants of health, and discharge planning/referral for Acute Medical and Surgical patients. Typical Caseload 20/day
Job Requirements:
3+ years Acute Care Case Management experience Active OH RN License Current BLS/CPR Certification (preferably from AHA) Fully Vaccinated against Covid-19 Who We Are?
