FULL TIME
Weekend Warrior Registered Nurse Case Manager (RNCM) for the Youngstown area. This position will respond to after-hours clinical/non-clinical situations, perform triage, conduct home visits, and update care plans for hospice and home health patients. This position will assist in providing compassionate, holistic end-of-life care for hospice patients, focusing on comfort, dignity, and quality of life. The Registered Nurse (RN) will manage pain/symptoms, educate families, coordinate with an interdisciplinary team (doctors, social workers, chaplains), and offer emotional/spiritual support in homes, facilities, or hospice centers.

Responsibilities:
Supports the hospice process and serves as the liaison between the patient and the IDG. Performs initial and ongoing assessment of the terminal diagnosis on the patient's physical, functional, psychosocial, environmental needs and activities of daily living. Provides end-of-life, palliative, and specialized care and counseling related to terminal illness including patient/representative education on disease process, nutritional support (if qualified), and symptom management. Supervises LPN and aides' services assigned to his/her patients Secures pharmaceuticals, durable medical equipment, supplies, and other community services related to the hospice plan of care for assigned patients. Assesses and manages patient pain levels, by providing symptom relief. Uses agency documentation materials and/or computer-based software to record assessment findings and patient-care Meets productivity requirements established for hospice case management. Assesses caregiver's ability to manage patient needs and consider respite or alternative options, if needed. Evaluates the safety of the home environment and recommends to patient/representative potential solutions for identified concerns.
Qualifications:
Must be a graduate of an accredited school of nursing Must possess two years of experience in hospice setting Possess and maintain a current state licensed and in good standing. Maintain CPR certification Possesses good time management, health assessment, and work ethic skills Possesses self-direction and ability to make independent patient care judgments Presents a professional appearance and promotes a positive work environment Demonstrates computer literacy Possess and maintain current driver's license and have a dependable, insured automobile
