Position Summary The MDS Coordinator is responsible for coordinating and completing the MDS/RAI process while helping ensure accurate resident assessments, individualized care planning, regulatory compliance, and appropriate reimbursement. This position works closely with nursing, therapy, administration, and the interdisciplinary team. Key Responsibilities Coordinate and complete MDS assessments accurately and within required timeframes. Manage the RAI process and maintain compliance with CMS requirements. Coordinate with the interdisciplinary team to ensure comprehensive and accurate resident assessments and care plans. Monitor assessment schedules, ARDs, and completion deadlines. Review clinical documentation for accuracy and support of resident acuity and reimbursement. Maintain knowledge of PDPM, Medicare requirements, and changes affecting MDS and reimbursement. Participate in Medicare meetings, IDT meetings, Triple Check, QAPI, and other clinical/operational meetings as needed. Work collaboratively with nursing, therapy, medical records, business office, and facility leadership. Assist with survey readiness and ensure MDS-related documentation meets regulatory requirements. Identify opportunities to improve documentation, quality measures, and resident outcomes. Qualifications Current Oklahoma nursing license required and previous MDS experience in a skilled nursing facility . Candidates should have a working knowledge of the RAI process, PDPM, Medicare, care planning, and long-term care regulations. RAC-CT certification is a plus but not required. Why Elmbrook Home?
