Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members Review and evaluate patients previous and current medical histories, conditions, and medication regimens Educate patients on their health conditions and prescribed medications Recognize and document any gaps in care, providing recommendations to address these issues Focus exclusively on assessments no need to prescribe medications or develop treatment plans

Qualifications:

Active Nurse Practitioner license in the State of Oklahoma Current CPR (BLS) certification Board certification through AANP or ANCC Minimum of 6 months experience as a nurse practitioner preferred Master of Science in Nursing (MSN) from an accredited program This role offers competitive compensation, with $80$90 per evaluation plus mileage reimbursement and an administrative time stipend.
