Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members Review prior and current medical histories, chronic conditions, and medication records Educate patients on managing medical conditions and medication usage Identify any gaps in care and offer effective recommendations to address them No requirements for creating treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner License in Oklahoma Valid CPR certification (BLS) Board certified through AANP or ANCC A minimum of 6 months recent NP experience is preferred MSN degree earned from an accredited nursing program This per diem role offers highly competitive compensation of $80$90 per evaluation, plus a mileage stipend and coverage for administrative time.
