Deliver comprehensive healthcare assessments as a Nurse Practitioner in a per diem home health setting. Responsibilities:

Perform annual health risk assessments during 45-minute home visits Review and document patients current and prior medical history, medical conditions, and medications Deliver patient education focused on conditions and prescribed medications Identify gaps in care, offering actionable recommendations for improved outcomes Visits may include annual wellness assessments or post-discharge follow-ups

Qualifications:

Active Nurse Practitioner license in Oklahoma Current BLS/CPR certification Board-certified through either AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution In this role, your expertise will directly contribute to improving patient quality of life and ensuring members have the resources and knowledge they need to manage their health effectively.
