Job Description

Job Overview We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation process for long-term care residents, ensuring compliance with federal and state regulations such as CMS standards. Your expertise will facilitate accurate clinical documentation, optimize patient care planning, and support reimbursement processes. The ideal candidate will possess strong clinical knowledge, proficiency with electronic health record (EHR) systems, and experience in long-term care or skilled nursing facilities. This position offers an exciting opportunity to impact patient outcomes through meticulous data management and interdisciplinary collaboration. Duties Conduct thorough assessments of residents' clinical conditions using the MDS framework, ensuring all documentation aligns with clinical standards and regulatory requirements Coordinate and facilitate interdisciplinary team meetings to review resident data, care plans, and discharge planning strategies Review medical records, clinical documentation, and coding details—including CPT (Current Procedural Terminology), ICD-9, ICD-10, DRG (Diagnosis-Related Group), and HEDIS metrics—to ensure accuracy and completeness Maintain compliance with healthcare regulations such as HIPAA, state healthcare laws, Medicare regulations, and long-term care policies Perform documentation review to identify gaps or inconsistencies in medical records and coordinate corrective actions Support quality improvement efforts by analyzing HEDIS scores, CMS audits, and other performance metrics to enhance clinical documentation standards Qualifications Proven experience in long-term care facilities or nursing homes with a strong understanding of MDS processes and regulations Knowledge of healthcare coding systems including
ICD-9, ICD-10, CPT
coding, DRG classifications, and medical terminology Familiarity with EMR/EHR systems Experience with utilization review, case management, discharge planning, and clinical documentation improvement programs Strong understanding of Medicare regulations, CMS guidelines, HIPAA compliance, and state healthcare policies Excellent organizational skills with the ability to manage multiple priorities efficiently Effective communication skills to collaborate across multidisciplinary teams and ensure accurate documentation standards Join us in making a difference by ensuring our residents receive the highest quality of care through precise data management and regulatory compliance.
