VitalCaring is seeking a compassionate Registered Nurse to deliver high-quality home health and hospice care. In this role, you will complete comprehensive assessments, coordinate individualized care plans, and provide skilled nursing interventions- including medication administration, wound care, and symptom management- in the patient's home. Responsibilities Provide skilled nursing care to home health and hospice patients in their homes Complete comprehensive assessments and develop individualized care plans Administer medications, treatments, and wound care per physician orders Document care accurately in electronic health records and communicate with the care team Educate patients and families on disease management, medications, and safety Coordinate with physicians, therapists, social workers, and caregivers to optimize outcomes Monitor patient conditions, respond to changes, and escalate concerns promptly Support patients and families with compassionate end-of-life and symptom management care Required Skills Patient assessment Care planning IV therapy and medication administration Wound care Electronic health records (EHR) documentation Care coordination and case management Home health nursing practices Hospice and palliative care principles Infection prevention and control Patient and family education
