Base salary plus multiple incentive opportunities

Additional compensation available for providers who are fluent in a language other than English and use those skills during member visits Travel & Location Travel throughout the state of Oregon (approximately 85% travel)

All travel expenses covered

Providers can live anywhere in Oregon Work-Life Balance Flexible scheduling with hours between 8:00 AM and 8:00 PM

No on-call responsibilities

No holidays required About the Role Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes. Key Responsibilities Conduct comprehensive in-home health assessments

Review medical history and medications

Perform physical exams, screenings, and point-of-care testing

Identify and document diagnoses for care planning and treatment management

Communicate findings to primary care providers

Educate members on disease management, medications, and preventive care

Address social determinants of health and connect members with community resources

Recognize and respond appropriately to urgent or emergent situations

Required Qualifications NP:

ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

PA:

NCCPA certification and active state licensure

Active, unrestricted license (or ability to obtain)

Valid driver's license and reliable transportation

Ability to work in a variety of home environments and travel extensively Preferred Qualifications 1+ years of clinical experience in family practice, geriatrics, primary care, or home health

Experience with Medicare or geriatric populations

EMR and technology proficiency

Strong communication and patient education skills

Fluency in a language other than English Highlights Statewide Oregon travel role with expenses fully covered

$40K Sign-On Bonus or $40K Student Loan Repayment

Flexible schedule and autonomy while providing meaningful one-on-one patient care in the home setting. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
