Job Description

Seeking a skilled Nurse Practitioner to deliver comprehensive home health risk assessments on a per diem basis within Clackamas County and surrounding areas. This flexible opportunity allows you to create your own schedule, with shifts available any day from Monday through Sunday. Assignments may include annual health assessments or post-discharge visits, typically lasting about 45 minutes, for Medicare Advantage plan members residing in areas such as Lake Oswego, Oregon City, Tualatin, and West Linn.
Schedule:
Self-scheduled, with shifts available Monday through Sunday Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
Conduct annual health risk assessments and post-discharge visits for Medicare Advantage plan members in their homes Review prior and current medical history, assess medical conditions, and evaluate medication history Educate members on their health conditions and current medications Identify gaps in care and present actionable recommendations for follow-up No prescribing medications or creating treatment plans required
Qualifications:
Active Nurse Practitioner license in Oregon Board Certification through AANP or ANCC Current BLS CPR certification MSN degree from an accredited nursing program At least 6 months of recent experience as an NP preferred
Compensation:
This role offers $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
