Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage plan enrollees Review and evaluate patients prior and current medical histories, medical conditions, and medication usage Educate patients about their health conditions and medications to empower self-care Recognize gaps in care and offer actionable recommendations, enhancing care quality No requirement to create treatment plans or prescribe medicationsfocus on evaluation and education

Qualifications:

Active Nurse Practitioner license in Oregon Current BLS/CPR certification Board certification from AANP or

ANCC MSN

from an accredited nursing school At least six months experience as a Nurse Practitioner preferred

Additional Incentives:
