Youll serve Medicare Advantage members in Bend and neighboring towns such as Redmond, Deschutes River Woods, Three Rivers, La Pine, and Sisters, providing personalized care in patients homes.

Key Responsibilities:

Perform comprehensive, annual home health risk assessments for Medicare Advantage plan members. Review and evaluate patients past and current medical history, medical conditions, and medication usage. Deliver education to patients about their health conditions and prescribed medications. Identify gaps in care and offer recommendations to improve health outcomes. Visits are limited to assessmentno prescribing or treatment planning required. Typical assessments last approximately 45 minutes each.

Qualifications:

Active Nurse Practitioner license in Oregon. Current BLS certification (CPR). Board Certification with AANP or

ANCC. MSN

degree from an accredited nursing school. At least 6 months of current Nurse Practitioner experience preferred.
