Key Responsibilities:

Conduct risk assessments for Medicare Advantage health plan members in their homes Review and evaluate patients medical histories, existing conditions, and medication regimens Educate patients about their medical conditions and prescribed medications Identify gaps in care and recommend actionable steps to address them No requirement for developing ongoing treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner License in the state of Oregon Active CPR certification (BLS) Board Certification through AANP or

ANCC MSN
