Deliver vital care and personalized service as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members across Jefferson County, OR. This per diem opportunity offers autonomy and flexibility, allowing you to design your own schedule and work in diverse home settings throughout Madras, Culver, and Metolius.

Schedule & Hours:

Flexible shifts available Monday through Sunday Design your own schedule based on availability Minimum weekly or bi-weekly hourly commitment

Responsibilities:

Perform annual health risk assessments in members homes, primarily focused on comprehensive evaluations Review medical history, chronic conditions, and medication regimens Educate patients on health conditions, recommended screenings, and proper medication management Identify health care gaps and make appropriate recommendations to address these needs Visits typically take 45 minutes and may include annual evaluations or post-discharge check-ins No requirement for prescribing medications or developing treatment plans

Qualifications:

Current Nurse Practitioner license (NP) in Oregon Board Certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited program Active CPR certification (BLS) At least 6 months of Nurse Practitioner experience preferred Excellent communication and interpersonal skills, with the ability to provide clear patient education
