Pay:
$42.00-$51.21/hour
Registered Nurse Case Manager Opportunity:
Full-time, permanent position Monday-Friday, working 8:00 AM-5:00 PM Working in either Home Health or in Hospice Rotating on-call every holiday The Registered Nurse Case Manager will collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans the Registered Nurse Case Manager will coordinate services that align with patient needs, payer guidelines, and clinical goals to ensure seamless care transitions
Registered Nurse Case Manager Requirements:
Registered Nurse License Required Atleast one year of case management experience working in a clinical setting
