Job Description

Lead clinical excellence in home health nursing by overseeing patient care and guiding team performance as a Home Health Clinical Manager. In this vital position, youll supervise Registered Nurses and care professionals, ensuring quality, safety, and compassionate service for patients transitioning between hospital and home settings. Your expertise will play a key role in maintaining standards and fostering a supportive atmosphere for both staff and families.
Schedule/Hours:
Contract position, flexibility may be available depending on facility needs
Qualifications:
Current, unrestricted RN license Proven experience in home health care as a Registered Nurse Prior supervisory or management experience in a clinical setting preferred Strong understanding of home health regulations, protocols, and best practices Excellent communication, leadership, and organizational skills Ability to collaborate across multidisciplinary teams and foster professional development Commitment to patient-centered care and continuous quality improvement
Responsibilities:
Oversee and support clinical staff to ensure compliance with home health guidelines and individualized patient care plans Conduct patient assessments, care planning, and case conferences as needed Monitor quality assurance, performance improvement, and report outcomes to senior leadership Provide education, training, and mentorship to RN team and allied health staff Foster strong relationships with patients, families, and referral sources Maintain accurate records, supervise documentation, and participate in audits or accreditation processes as required
