Key Responsibilities:

Perform detailed home health risk assessments for Medicare Advantage members Review and document prior and current medical history, existing medical conditions, and medication regimens Educate patients about their conditions and prescribed medications to encourage better self-management Identify care gaps and recommend appropriate actions to ensure continuity and quality of care No requirement to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Oregon CPR certification (BLS) must be current Board Certification via AANP or ANCC is essential At least 6 months of recent NP experience in a clinical setting preferred MSN degree from an accredited nursing program You can expect compensation of $80$90 per completed evaluation, and youll receive an additional mileage stipend plus compensation for administrative time.
