Deliver essential home-based care by leveraging your expertise as a Nurse Practitioner. This per diem opportunity offers flexible scheduling options with shifts available any day between Monday and Sunday. Youll have the autonomy to design your own work schedule, meeting minimum weekly or bi-weekly hour requirements.

Key Responsibilities:

Conduct in-home risk assessments for Medicare Advantage members throughout Linn County, including locations such as Albany, Lebanon, and Sweet Home. Perform annual health evaluations and post-discharge visits, typically taking around 45 minutes per appointment. Review each members medical history, current conditions, and medication regimen. Educate patients regarding ongoing medical issues and the use of prescribed medications. Identify care gaps and recommend steps for better health management.

Qualifications:

Current licensure as a Nurse Practitioner in Oregon Active CPR certification (BLS required) Board Certification through AANP or

ANCC MSN
