Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, medical conditions, and a complete medication history Educate patients on their medical conditions and medications Identify care gaps and provide recommendations to address them Visits vary (annual health assessment or post-discharge), generally lasting about 45 minutes each No requirement for treatment planning or prescription writing

Qualifications:

Active Nurse Practitioner license in Oregon Current BLS/CPR certification Board Certification through AANP or ANCC Minimum of six months of NP experience preferred MSN degree from an accredited nursing program Earn $80-$90 per completed evaluation, with additional mileage stipend and compensation for administrative time.
