Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members in their homes. Review and evaluate members prior and current medical histories, medical conditions, and medication profiles. Offer education to patients regarding their medical conditions and medication usage. Identify and document gaps in care, providing recommendations to address concerns. Visits will not require treatment plan development or medication prescribing. Each in-home assessment typically lasts about 45 minutes, and duties may include annual visits or post-discharge check-ins.

Qualifications:

Active Nurse Practitioner (NP) license in Oregon Current CPR certification (BLS) Board Certification via AANP or ANCC Preferred six months of current NP experience Master of Science in Nursing (MSN) from an accredited institution
