Key Responsibilities:

Conduct thorough annual health assessments and post-discharge visits in patients homes Review and evaluate medical history, existing health conditions, and current medications Offer tailored education to patients about their diagnoses and medication regimens Identify and communicate care gaps, providing recommendations to improve patient outcomes Focus solely on assessment and educationno medication prescribing or treatment plan creation required

Qualifications:
