Core Responsibilities:

Perform in-home annual health risk assessments for Medicare Advantage members Review and evaluate patients current and past medical histories, including conditions and medication regimens Educate patients on their individual health conditions and medications, promoting wellness and improved self-care Identify possible gaps in care, offering recommendations for additional services or follow-up as needed

This role does not involve direct prescribing or treatment plan creation

Qualifications:

Active Nurse Practitioner License (OR) MSN from an accredited nursing institution Current CPR certification (BLS required) Board Certification via AANP or ANCC Minimum of 6 months experience working as a Nurse Practitioner preferred
