Key Responsibilities:

Perform home-based annual risk assessments for Medicare Advantage plan members Conduct thorough reviews of prior and current medical histories, chronic conditions, and medication use Educate patients about their diagnoses, care needs, and prescribed medications Identify and address care gaps, recommending solutions to optimize health outcomes Visits may vary from routine annual assessments to post-discharge evaluations (average duration: 45 minutes) Prescribing and treatment plans are not part of this role

Qualifications:
