Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate current and prior medical history, health conditions, and medication use Educate patients about their conditions and medications to promote understanding and compliance Identify and report gaps in care, offering practical recommendations for improvement No need for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Oregon Board certification from AANP or

ANCC MSN
