Share Job Job Description RN East Portland Family Medicine Clinic 10000 S.E. Main St 1001 Portland, OR 97216 Community Health Center and Federally Qualified Health Center. Provide primary care for all ages and genders.
Required:
strong ambulatory epic in basket skills, phone triage and primary care or ambulatory clinic care coordinator experience. Wellness and prevention care Treatment for minor injuries and common illnesses Management of chronic conditions, like diabetes and high blood pressure Behavioral care Family Medicine at East Portland clinic is a primary care Family Medicine clinic and Federally Qualified Health Center (FQHC). The patient population includes all ages of the life spectrum including pregnant people, babies, children, adolescent, adults, and geriatric. The patients are generally moderately stable to moderately unstable, with or without complex acute or chronic illnesses and diseases, and SDOH barriers that require medical and nursing interventions. This role will focus primarily on telephone triage and advice, while also responding to patients' urgent needs in clinic or telephonically. Flexibility and a nimble attitude are key. Visit types offered at Family Medicine at East Portland are: In-person, virtual, and telephone.Services Offered:
- Nurse Telephone and in-clinic triage
- Behavioral health and counseling services
- Reproductive health services
- Transgender health care
- Prenatal and post-partum care (including deliveries)
- Pediatric care
- Medication-assisted treatment (MAT)
- Health maintenance with preventive care
REQUIRED
AHA BLS, BSN,- 2 years or more, within the last three years, experience working in direct patient care.
- Experience working in Primary Care or an FQHC, and/or Telephone Triaging
- Oregon and Washington State Nursing licensure upon hire
